American Association of Service Coordinators
Our mission is to advance the interests of the service coordinator (SC) profession and to provide guidance and professional standards to its members. Our vision is to support our members that serve individuals, families, the elderly and persons with disabilities in housing situations through leadership, education, training, networking, advocacy and other member services. AASC member Service Coordinators serve as a conduit to disseminate CHA and SMP fraud prevention information to their resident clients and host SMP educators at their facilities to conduct fraud prevention workshops.
ACC Senior Services
The mission of ACC Senior Services is to promote the general welfare and enhance the quality of life for our community by identifying, developing, and providing culturally sensitive health and social services for older adults. ACC staff and volunteers disseminate fraud prevention materials and act as a bridge to the CHA SMP program when beneficiaries have questions about their medical statements.
Benjamin Edward Jones Community Resource Center
The mission of the Benjamin Edward Jones Community Resource Center is to transform lives and our communities by providing continued services and support by enhancing the quality of life through programs, which promote culture education, and job placement services. BEJ Community Resource Center partners with CHA SMP via hosting Medicare fraud education sessions and disseminating fraud prevention materials to staff and senior program participants.
Bet Tzedek
Bet Tzedek is a nonprofit law firm located in the heart of Los Angeles. Our talented, passionate, and committed staff and volunteers provide free legal services, outreach and education, and policy advocacy for individuals and families across Los Angeles County and beyond. Bet Tzedek partners with CHA SMP in hosting a virtual Senior Symposium on topics of senior fraud and consumer protections, and in disseminating SMP fraud materials.
Blindness Support Services
Blindness Support Services is dedicated to teaching those who are blind or have visual impairments to become more independent and adapt to the challenges they have. Blindness Support Services partners with CHA SMP in providing feedback regarding our website for accessibility for those visually impaired.
California Advocates for Nursing Home Reform (CANHR)
CANHR is dedicated to improving the choices, care and quality of life for California’s long term care consumers. Through direct advocacy, community education, legislation and litigation it has been CANHR’s goal to educate and support long term care consumers and advocates regarding the rights and remedies under the law. CANHR and CHA SMP collaborate in sharing and disseminating each other’s consumer advocate and fraud materials online.
California Association for Adult Day Services
The California Association for Adult Day Services supports the development of adult day services as an alternative to institutional care and provides educational activities. CAADS disseminates CHA SMP fraud prevention materials through their electronic media.
California Association of Area Agency on Aging (C4A)
C4A is a nonprofit organization representing California’s 33 area agencies on aging and is the leading voice on aging issues in California. C4A works closely with the Administration on Aging, the California Department of Aging, Department of Social Services, Department of Rehabilitation as well as key provider associations. C4A and its members provide programs that enable older Californians to live independently for as long as possible. CHA SMP partners with C4A to do a Medicare session at its annual conference.
California Collaborative for Long Term Services and Support
The Collaborative is a coalition of statewide organizations serving seniors and persons with disabilities. These organizations have agreed to a set of principles and meet regularly to exchange information and to work toward consensus on the design of an integrated system of long term services and supports in California. The Collaborative also serves as a sounding board for the Legislature and the administration as they consider changes in services for seniors and persons with disabilities, from acute care hospitalization and nursing home care to home and community-based services. Government Action and Communication Institute provides staff support to the Collaborative. CHA is an active member of the Collaborative and shares SMP materials with collaborating organizations to share with their served populations.
California Department of Aging (CDA)
Under the umbrella of the California Health and Human Services Agency, the California Department of Aging (CDA) administers programs that serve older adults, adults with disabilities, family caregivers, and residents in long-term care facilities throughout the State. These programs are funded through the federal Older Americans Act, the Older Californians Act, and through the Medi-Cal program.
CHA collaborates with the CDA HICAP team to ensure that HICAPs receive the most up to date Medicare information for California beneficiaries. CHA also provides technical expertise and training presentations to CDA and its HICAP programs. CHA SMP works with CDA to disseminate SMP digital materials.

DHCS’ mission is to provide Californians with access to affordable, integrated, high-quality health care, including medical, dental, mental health, substance use treatment services and long-term care. DHCS funds health care services for more than 15.4 million Medi-Cal beneficiaries. DHCS and CHA SMP work together to disseminate each other’s materials, and help track and resolve statewide Medi-Medi issues, such as enrollment fraud and navigating the changes with CalAIM.

California Hospice and Palliative Care Association (CHAPCA)
CHAPCA is a not-for-profit organization with a mission to improve access to quality end-of-life care and educates patients, families, the community and healthcare professionals about hospice and palliative care. CHAPCA and CHA SMP collaborate on educational webinars and creating and disseminating education materials regarding Medicare hospice fraud.
California Indian Manpower Consortium, Inc.
The Consortium was created as a nonprofit for the purpose of working for the social welfare, educational and economic advancement of its member tribes, groups, organizations and Indians and other Native Americans living in its service area. CIMC and CHA SMP collaborate to educate tribal elders and their caregivers about Medicare and Medicare and healthcare fraud prevention.
California Long Term Care Ombudsman Association
The LTC Ombudsman are dedicated to improving the accessibility and quality of long-term care services for elders and vulnerable adults living in nursing homes, residential care facilities and other long-term care facilities across California. CHA SMP provides the CA LTC Ombudsman program with Medicare fraud prevention trainings and materials for dissemination.
California Rural Legal Assistance (CRLA)
CRLA is a nonprofit law firm founded in 1966 to provide free civil legal services to low-income residents of California’s rural counties. CHA SMP provides Medicare fraud training to staff and CRLA disseminates SMP materials and fraud alerts to their clients.
Center for Medicare Advocacy
The Center for Medicare Advocacy’s (CMA) mission is to advance access to comprehensive Medicare coverage and quality health care for older people and people with disabilities by providing exceptional legal analysis, education, and advocacy. CMA and SMP collaborate on webinars about Medicare and healthcare fraud prevention.
Center for Medicare and Medicaid Services (CMS)
The Centers for Medicare & Medicaid Services, CMS, is part of the Department of Health and Human Services (HHS) and administers and oversees Medicare and Medicaid programs and policy. CHA SMP collaborates with CMS in disseminating timely Medicare updates and fraud prevention education statewide, presenting at state conferences, and disseminating updates via CMS’ statewide newsletter to congressional aides.
Community Action Agency of Butte County
Community Action Agency of Butte County provides hope to people struggling with poverty by meeting tangible needs in the areas of energy savings, affordable food, and housing. CHA SMP provides Medicare fraud education to staff and fraud education and outreach materials for distribution.
Community Action of Napa Valley
Community Action changes people’s lives, embodies the spirit of hope, improves our community and makes Napa Valley a better place to live via its food bank, childcare and meals on wheels programs. Community Action of Napa Valley partners with CHA SMP to distribute SMP fraud prevention materials at their food banks and during meals delivery to older adults.
Community Action Partnership of Kern
Community Action Partnership of Kern (CAPK) administers more than a dozen programs, including health and nutrition services, services to families and caregivers aimed at meeting children, families and older adults at their point of need, for those who have had difficulty engaging the “American Dream.” CHA SMP provides Medicare fraud education to staff and fraud education and outreach materials for distribution.
Community Food Bank of San Benito
Community Food Bank provides thousands of pounds of nutritious supplemental food to their neighborhoods. Their mission is to build dignity, share abundance and nourish lives. Community Food Bank distributes CHA SMP fraud prevention materials in food bank boxes and bags.
Conejo Valley Senior Concerns
Serving the community since 1975, Senior Concerns is a private, non-profit organization, dedicated to serving the senior community and especially frail and special-need seniors, caregivers and families by providing quality programs, appropriate resources and educational outreach, raising funds and developing and administering quality-of-life programs for seniors with special needs. Conejo Valley Senior Concerns partners with CHA SMP in distributing SMP fraud education materials to their community members and providing presentations.
Emergency Food Bank
The mission of the Emergency Food Bank is to build a stronger community by providing a safety net of healthy food, nutrition education, and hope to residents of San Joaquin County who find themselves in need. Emergency Food Bank distributes CHA SMP fraud prevention materials in food bank boxes and bags.
Family Service Association
For nearly 70 years, Family Service Association has addressed poverty, hunger, and health, from infancy to older adults, in families and under-served communities. FSA’s motto is compelling, Family Strength Is Community Strength. FSA partners with CHA SMP in sponsoring educational presentations to communities and distributing SMP fraud education materials.
Federal Trade Commission
The FTC’s mission is protecting the public from deceptive or unfair business practices and from unfair methods of competition through law enforcement, advocacy, research, and education. CHA SMP partners with FTC in disseminating pertinent fraud alerts and presenting at annual conferences.
Fort Bragg Food Bank
The Fort Bragg Food Bank is a nonprofit 501(c)(3) corporation aiming to distribute food to those who need it most. Fort Bragg Food Bank distributes CHA SMP fraud prevention materials in food bank boxes and bags.
Foundation for Senior Services
The Senior Specialists Group chapters get involved in their local communities to offer education, information and quality services on every level of senior care. The Foundation provides opportunities for CHA and SMP to conduct education through its many chapters and community events.
Health Education Council
The Health Education Council exists to improve health outcomes in under-served Sacramento neighborhoods by empowering the people in them to create the solutions they need most. HEC partners with CHA SMP in disseminating SMP fraud materials to older adult participants and family members in communities they serve.
Health Insurance Counseling and Advocacy Program (HICAP)
The HICAPs provide free and objective information and counseling about Medicare. HICAP also offers free educational presentations to groups of Medicare beneficiaries, their families and/or providers on a variety of Medicare and other health insurance-related topics. HICAP staff and volunteers educate Medicare and Medi-Cal beneficiaries about healthcare fraud prevention throughout the state.
Hope Through Housing Foundation
Hope through Housing is dedicated to breaking the cycle of generational poverty by implementing programs that empower individuals and change communities. They deliver services and support to low-income residents within National CORE apartment communities aimed at Building Bright Futures, creating Pathways to Economic Empowerment, and building Connections to Care for seniors. CHA SMP partner with HTHF through distributing SMP fraud education materials to those working in the Connections to Care program with seniors.
Inland Empire Caregiver Resource Center
Inland Empire Caregiver Resource Center helps families and communities cope with and manage the challenges of aging and caregiving through caregiver support services, supports for seniors living alone, and services to address social isolation and depression among older adults. Inland Empire Caregiver Resource Center partners with CHA SMP via fraud detection and Medicare summary notices review training for caregivers, training for staff on fraud prevention and dissemination of fraud alert and other SMP materials to caregivers and seniors.
Janet Goeske Foundation
Janet Goeske Foundation is committed to enriching the quality of life of those 50 & older in the Riverside community they serve while maintaining the highest level of advocacy and providing health & wellness programs. Janet Goeske Foundation partners with CHA SMP in hosting fraud prevention education presentations and disseminating SMP materials at health and wellness programs.
Life Skills Training and Education Programs (LifeSTEPS)
The mission of LifeSTEPS is to provide effective educational and supportive services to maximize the strengths of individuals and build resilient communities. LifeSTEPS partners with CHA SMP to share fraud prevention information to older adults and their families and serves as a bridge with SMP to assist residents with questions and concerns about their medical statements.
Madera County Food Bank
The Madera County Food Bank has been serving the food needy of Madera County since 1999. Since then they have partnered up with many county agencies that have helped them grow the Food Bank. They now run five thriving programs to fulfill the needs of their communities. With the help of all sixteen of their EFAP distribution sites and twenty Senior Brown Bag sites they strive to get food out to all the families in need. Madera County Food Bank distributes CHA SMP fraud prevention materials in food bank boxes and bags.
Meals on Wheels Orange County
Today, one in four low-income Orange County older adults struggles with hunger. In addition, one in four older adults in the county are at greater risk of death due to loneliness. Our mission is to address this by nourishing the wellness, purpose and dignity of older adults and their families in our community by providing meals and socialization in addition to other programs to combat hunger and isolation. MoW distributes CHA SMP fraud prevention alerts during meals delivery.
Office of Inspector General (OIG)
The Office of Inspector General (OIG) has been at the forefront of the Nation’s efforts to fight waste, fraud and abuse and to improving the efficiency of Medicare, Medicaid and more than 100 other Department of Health & Human Services (HHS) programs. CHA SMP collaborates with the OIG in disseminating timely their fraud alerts and consumer protection tips and collaborating/presenting at state conferences, and complex case referrals.
ONEgeneration
Their mission is to enrich the lives of older adults and families throughout our diverse communities. ONEgeneration currently serves over 5,000 older adults and their family caregivers in Los Angeles county annually through our range of aging services with the purpose of keeping older adults active, independent and remaining at home as they age in place safely. ONEgeneration staff and volunteers distribute CHA SMP fraud prevention materials through multi-media and sponsor SMP to conduct educational webinars about fraud.
Salvation Army Silvercrest
The Salvation Army is dedicated to doing the most good in the communities they serve, and bases their services on what each specific community needs. They provide community centers in many communities. Salvation Army partners with CHA SMP in training staff/volunteers in Medicare fraud and current trends, hosting presentations at community centers and distributing fraud education materials.
San Manuel Band of Mission Indians
The San Manuel Band of Mission Indians is a federally recognized American Indian tribe located near the city of Highland, Calif. San Manuel is one of several clans of Serrano Indians, who are the indigenous people of the San Bernardino highlands, passes, valleys mountains and high deserts who share a common language and culture. CHA partnered with San Manuel and its Indian Health Services clinics to educate clinic staff and benefit counselors on Medicare.

Today’s Woman Foundation’s mission is to empower our diverse community of women through education, advocacy, and resources to enhance body, mind, and spirit. TWF partners with CHA SMP in distribution SMP fraud prevention materials to the women in their main programs.
Valley Interfaith/Intercommunity Council
Our mission is to create positive change within the communities in the Greater San Fernando Valley as we serve, support and engage others through unity and faith in action, enriching lives and strengthening communities. VIC partners with CHA SMP to distribute SMP fraud prevention materials during meals delivery to older adults.
Village Movement California
The mission of our organization is to transform the experience of aging across the state by strengthening impact, accelerating growth, and ensuring sustainability of the Village Movement. Village Movement California advocates for innovative, community-based solutions to improve quality of life and expand choices at all stages of aging, empowering older adults to sustain independence through community and remain in charge of their lives as they age. CHA SMP provides fraud prevention education to the members of its approximately 40 Villages. The Village Movement identifies Ambassadors in individual Villages to liaise with CHA SMP.

Wise and Healthy Aging provides a wide range of elder services with a focus on the Los Angeles Westside and the surrounding communities within the greater City and County of Los Angeles.
Yocha Dehe Wintun Nation
Yocha Dehe is an independent, self-governed nation that supports our people and the Capay Valley community by strengthening our culture, stewarding our land and creating economic independence for future generations. CHA partnered with Yocha Dehe to do outreach and education on Medicare benefits to elders and caregivers of Yocha Dehe and surrounding tribes.

Yolo Food Bank increases food and nutrition security and helps create an equitable and sustainable local food system by: connecting individuals and families to healthy, high-quality food and resources; collaborating with partners to deepen outreach & address food needs; and convening key players in the community to assess progress & plan collective actions.
